She is a Nurse Care Manager Supervisor from the Indiana Primary Healthcare Association (IPHCA) which exists to ensure that all Indiana residents have a "health care home" which provides quality care regardless of economic status or background. It was organized in 1982 and is a statewide not-for-profit advocacy organization whose mission is to promote the provision of primary health care services in areas of need in Indiana and to assure the growth and development of Community, Migrant, and Homeless Health Centers in the State.
MISSION:
To advocate for access to quality health care for all those residing in Indiana and to support the development of community oriented primary care initiatives, which are:
Affordable: based on income
Available: when needed
Accessible: where needed
Appropriate: emphasizing continuous, comprehensive, prevention-oriented primary care and
case management
Acceptable: to all residing in the community, regardless of cultural heritage, financial status, or personal circumstances
GOALS:
To Influence federal, state and private policies and resource allocations to achieve IPHCA's mission.
To increase the number of culturally competent primary care clinicians and services in medically underserved areas.
To facilitate the development of new starts and/or the expansion of existing Health Centers for underserved populations.
(With members) To initiate strategies to thrive within the changing health care environment.
To Support/promote/advertise continuous quality improvement within IPHCA's membership.
To create a non-competitive environment for primary care entities.
To initiate and maintain an ongoing IPHCA Board development strategy.

Debbie works with Chronic Disease Management and High Risk Management of Medicaid. SHe is very knowledgeable about teaching self managment skills. Debbie will teach you to take ownership of your diseaseand ways to work better with your physician.
